    Before you come in, we ask that you print and fill-out these forms so we can expedite the paperwork and begin focusing on getting to know each other.

    Insurance & Billing

     We accept most insurance providers including ARKids. We are a provider for Cigna, Delta Dental, and Blue Cross Blue Shield. We can also assist you arrange third-party financing.

    Payment Options

     We accept checks, cash, Visa, MasterCard, Discover, American Express, Care Credit and Wells Fargo.
